Exercise Referral
Lincs Inspire’s exercise referral scheme – Active Forever – is a health and wellness programme designed to help you take the necessary steps towards a lifestyle change that can lead to lower medication, less pain, and an increased quality of life.
Available to all residents over the age of 18 who are living with a health condition or undergoing rehabilitation. The specialist scheme leads to improved physical and mental wellbeing and helps you to manage your symptoms, prepare or recover from surgery.
Active Forever is a 12-week scheme with patients being referred by their health professional (i.e. practice nurses, health trainers, GPs or physiotherapists).
The supported scheme may be free to access, depending on your medical condition.
You can speak directly to your health professional about our scheme, or you can also express an interest in Active Forever by completing our form. We will then send you further information that you can share with your health professional.
About Active Forever
Active Forever will support you in your first steps to a more active and healthier lifestyle.
Our friendly and specialist staff will guide you through the 12-week wellness scheme, where small changes can make a big difference. Two activities are recommended per week and we work closely with you to ensure these are suited to your abilities and needs. Activities can include:
- Use of the latest power-assisted therapy equipment in the Wellness Hub
- Gym sessions in any of our four gyms (including specialist adapted equipment at Grimsby Health & Wellbeing Centre)
- Swimming in any of our three pools
- Group exercise classes across any of our four leisure sites
- The Reading Agency's Reading Well, supported by Lincs Inspire Libraries
- Healthy Weight Management sessions
- Coming soon - fitness sessions in the community such as Tai Chi, Kurling and low impact Zumba.
Please note: not all activities are available at all sites.

How much does Exercise Referral cost?
Active Forever costs £54 for 24 sessions to cover the 12-week programme.
NEW! Depending on your medical condition, you may qualify for a fully funded place on our exercise referral scheme, available for a short time only due to Government funding. See if you qualify before completing our form for further information.
Which health conditions does the programme cover?
The programme covers a wide range of health conditions such as:
- Asthma
- Arthritis
- High blood pressure
- Diabetes
- Obesity
- Depression, stress and anxiety
- Osteoporosis
- Neurological conditions - Parkinson's Disease, Multiple Sclerosis
- Cardiovascular conditions - Stroke and Hypertension

How does the Active Forever help?
When managing a long-term health condition, being active can often help you to manage your symptoms more effectively. Furthermore, if you are awaiting surgery or treatment for a health condition, pre-habilitative activity can help to make a significant difference to the ability to cope with treatment and support a swifter recovery.
It can often be very difficult to find the right type of exercise, especially when you are managing pain and are not used to regular physical activity.
Our Active Forever scheme provides access to the latest therapy equipment, low impact exercises, social activities, as well as support from experienced and specialist wellbeing practitioners.
What happens after I have finished Active Forever?
After you have completed Active Forever, we encourage you to continue your healthier lifestyle with us and you have the option to sign up as a member at a concessionary rate (£25 per month).
You have a month to benefit from this reduced membership price; after this month has lapsed, standard membership rates apply.
